Summary - The deceased was a 19 year old female who was an involuntary patient at Graylands Hospital and who died on 13 October 2012.

The inquest was held together with the deaths of nine other persons who had been held in care as involuntary patients at Graylands Hospital immediately before they died.

The Coroner found the deceased died on 13 October 2012 at Graylands Hospital from combined drug effect and myocarditis. The manner of death was misadventure.
