Inquest into the Death of Nola Inez Daniel

Delivered on 23 July 2014 at Perth.

Finding of Coroner Linton.

Summary

The deceased was an 83 year old at the time of her death and in the care of the Regis Forrest Gardens, an aged care facility in Bunbury.

The inquest focused primarily on the quality of the care provided after each fall during her brief stay at Regis Forrest Gardens.

The inquest examined the decision of the staff of the aged care facility not to call a doctor or take the deceased to hospital on each occasion, as well as identifying the particular circumstances in which the deceased came to fracture her hip before she was taken to hospital.

The Coroner found that the deceased died on 7 January 2010 at Bunbury Regional Hospital as a result of complications following fracture and surgical repair of the left hip in an elderly lady with underlying chronic lung disease and valvular heart disease. The Coroner found that death arose by way of accident.
